Department of Dermatology

The Section of Dermatology of the University of the Philippines – Philippine General Hospital has been in existence since 1973. It is a community of scholars whose mission is to attain excellence, respect and relevance in Dermatology through the development of competent dermatologists who serve the people, not for material gain but because of genuine concern for their welfare.

The vision of the section is to produce future dermatologists who are not only academically excellent, but also service-oriented, research-inclined, community- and civic-minded, willing and dedicated to impart knowledge, skills and proper attitudes; and potential leaders committed to the development of the Philippine society. Furthermore, it advocates the development of specialists imbued with moral, ethical and spiritual rigor inspired by love, compassion and respect for the dignity of human life.

The section has eleven highly competent consultants all of whom are active faculty members of the UP College of Medicine . In fact, the consultant staff consistently tops the evaluation of teachers by the students in the undergraduate levels.

The section has fifteen regular slots for residents in training. The three-year residency program is accredited by the Philippine Dermatological Society. Its objectives are to develop knowledge and skills in the systematic approach to the diagnosis and treatment of skin diseases, to acquire basic research techniques and undertake research studies relevant to skin diseases. Professional ethics and good interpersonal relationships with patients, students and colleagues are also emphasized.

The program includes actual case recognition and management, case presentations, lecture discussions, and demonstrations of dermatologic diseases encountered in the clinics, hospital and community. It also includes teaching, training and mentoring of undergraduate medical students from first year medicine proper to internship, discussions on patient-doctor interrelationships, diagnostic procedures, therapeutic intervention, preventive measures and research.

Each year, about 25,000 patients are seen at the outpatient department. The residents also see dermatology referrals in the wards, emergency room and intensive care units. Owing to the large volume of cases seen each year, clinical material is abundant. Residents and fellows are assured of excellent training as their mentors are not only recognized in their fields, but are dedicated teachers as well.

There is a one-year fellowship program in Dermatopathology accredited by the Philippine Dermatological Society. Its objective is to develop adequate knowledge and skills in the histopathologic diagnosis of cutaneous disorders.

The section, in partnership with the Medical Social Services Division of PGH, established the Psoriasis and Hansen’s Clubs, which are support groups for patients afflicted with either Psoriasis or Hansen’s Disease. Each group has more than 30 active members who meet twice a month. The bimonthly meetings include lectures on the clinical nature and management of these diseases, discussions on nutrition, stress management and building self-esteem, as well as livelihood training seminars.

A postgraduate course in dermatology is organized annually by the section, with the special feature of being held in different provinces of the country each year. For 15 years, it has succeeded in bringing new concepts in dermatology to colleagues throughout the country.

Research Institute for Tropical Medicine of the Department of Health

The Section of Dermatology of the Research Institute for Tropical Medicine (RITM) of the Department of Health has undergone considerable growth in striving to keep pace with the community’s dermatologic care needs. Through the support, encouragement and unwavering assistance of the Medical Director of RITM, Dr. Remigio Olveda, the section has achieved its objectives of rendering the highest quality service to patients, providing an exceptional training program for its residents and conducting researches which aim to address public health concerns.

The dermatology section is under the Medical Department whose head is Dr. Rose Aplasca de los Reyes and the Medical Department is under the Clinical Division of RITM whose Chief is Dr. Beatriz Quiambao.

The section was founded in 1992, through the initiative of Dr. Gertrude P. Chan, who envisioned an ideal training program. Dr. Ma. Teresita Gabriel was the Section’s Training officer and Dr. Ma. Luisa Venida as consultant both shared her aspirations. Upon fulfillment of the requirements of a training program, the section was conferred full accreditation by the Philippine Society for Cutaneous Medicine (PSCM) in 1993. That same year, Dr. Flordeliz Casintahan was the section’s dermatopathologist.

Consultants and residents actively participate in local and international scientific meetings and conferences. The dynamism that has emanated from our residents enabled them to garner awards and gain distinction.

The current residency training program is designed to prepare the physician for a career in dermatology and its various subspecialties. The emphasis is to develop excellence as a clinician which will allow the graduate not only to be competitive in their field, but to attain leadership positions in the community, as well as provide unconditional service to indigent patients.

OUR VISION STATEMENT

In the methodological course of sixteen (16) years towards the turn of the millennium, the infrastructural capability of the RITM’s section of Dermatology was built through international and locally funded collaborative research works essentially in the detection, diagnosis, treatment and control on leprosy- saw in Culion the dramatic decrease of the prevalence rate of 290/10,000 population in the 1985 to less than 1/10,000 population in 2001, and the publication of said collaborative research works- 10 international, 7 local and 23 paper presentations in both foreign and local fora, and likewise, in collaboration with Culion Foundattion, Inc., with the aid of its illustrative reference book “On the Diagnosis and Treatment of Common Skin Disorders and Leprosy,” lectured and trained Municipal Health Doctors and workers from Tawi-Tawi, Siquijor, Bohol and Cebu, the 1992 initiated Dermatology Training Program to date has some 50 graduates paracticing dermatology nationwide.

Given our initial 16 years of historical dynamism, we the consultants, residents, and staff of RITM’s section of Dermatology will in synergy pursue to do justice with our legacy by measuring-up with dermatology demands of our times and achieving distinction in this field of specialty

Ospital Ng Maynila Medical Center

Department of Dermatology

The Section of Dermatology of the Ospital ng Maynila medical Center was established in 1988 under the Department of Internal Medicine. From a one resident- one consultant-one cubicle room in the OPD, the section has grown in leaps and bounds into the premiere skin, hair and nail referral center of the city of Manila. Dr. Benedict L. Carpio, a fellow of the Philippine Society of Cutaneous Medicine, heads it. The stringent residency training program is supervised by Dr. Eileen Regalado-Morales, the first head of the section, now the residency training officer. Drs. Marcellano Cruz. Dermatologic Surgeon, STD/HIV specialist; Jonathan Dizon, research coordinator; and Amelita Tanglao, dermatopathologist, make up the rest of the staff.

Residency in Dermatology at the Ospital in Maynila was accredited on December of 1995. The life of a derma resident entails a rigid schedule of daily OPD duties, bedside rounds, journal clubs, Fitzpatrick, Ackerman and Lever’s clubs, weekly grand rounds, histopathologic sessions, and never ending case presentations. Not to mention monthly examinations and scientific meetings with Philippine Society of Cutaneous Medicine and other courses. No wonder other hospital residents claim that derma residents perpetually have their noses buried in their textbooks. Residents going through the three year program are guaranteed quality supervisions in the various subspecialties such as medical mycology, histopathology, clinical dermatology, investigative dermatology, surgical and cosmetic dermatology.

In 2002, after thorough scrutiny and evaluation of the section’s capacity to stand on its own, and taking into consideration, the increasing number of patients and cases seen; the appropriate residents: to consultant ratio; the strict training program, the section was elevated to department status. Coinciding with this, in what seemed to be a lucky streak, all four graduates shared the honor and prestige of being specialty board top notchers.

With the unification of the two premiere dermatology societies in the country, the department and its training program was given full accreditation by the Philippine Dermatological Society.

In 2004, another consultant joined the department. Dr. Perlie P. Portugal, also a consultant and professor at the UERMMC, handles the immunodermatology lecture series .

It is the department’s mission- vision to train physicians in the art and science of dermatology as a medical specialty, while providing excellent as well as compassionate services to all patients, particularly Manilans, regardless of stauts, creed or religion.

East Avenue Medical Center

Department of Dermatology

Year 1992 marked the birth of the Dermatology Residency Training Program at the East Avenue Medical Center. That time a mere 10 square meter room with 1 desk and 3 chairs served as office & outpatient clinic for the first 3 residents & consultant staff which included Drs. Monina A. Macam, Ma. Angela M. Lavadia, and Marcellano S. Cruz. With the vision of training competent dermatologists and developing future leaders in the field, the Section of Dermatology embarked on this endeavor with great enthusiasm and dedication, even if faced with seemingly insurmountable task of providing good training with no visible source of funding. During our humble beginning, surplus from dessert storm were donated to us where we acquired our first surgical instruments and wooden crates which we used as tables. It was not until 1994 that we acquired our very own Dermatology office, Conference room, & Library. In 1995, the Section moved to the OPD building. January 1999 marked another milestone in our history…we have been acknowledged as a Department.

Through the years, our Department has grown. But the challenge does not end here. We were fortunate to be awarded a 256 square meter clinic. In December 2003, we transferred to the newly built Out-Patient Department, which houses the new Dermatology Center. This serves as the Dermatology Referral Center of Quezon City, as well as the surrounding cities of Metro Manila Luzon. The Center is presently divided into several units which include the General Out-Patient Clinic, Contact Dermatitis, Phototherapy, Dermatologic Surgery, Skin Cancer, Genodermatoses and Infectious Dermatology. The Out-Patient Unit is located in a hospital wing dedicated solely for dermatology use. This serves as the area where primary assessment of patients with skin disorders is made. It has an attractive, comfortable setting furnished with amenities to serve the patient as thoroughly as possible. Consultations are either seen at the out-patient clinic or the hospital wards on a daily basis.

In our perseverance to complete the Department’s physical plan and facilities, we have pooled our resources to come up with the necessary equipments to address the needs of the patients. We have been able to upgrade our facilities by annual fund raising projects such as movie premieres, bingo bonanzas, raffle draws and acoustic nights. We have been able to acquire several instruments and equipment, including a Daavlin Phototherapy machine & a 5-headed microscope. The Departmental Library houses text books, journals, videos and discs in dermatology and allied disciplines. Computers with extensive graphics and word processing capacity are dedicated to trainee use.

The Phototherapy Center has benefited many psoriasis patients since its inception twelve years ago. Patients with psoriasis and other light responsive conditions are treated with UVB or PUVA in a manner which allows them to live normal lives. A convenient location and flexible appointment times ensure the highest quality of care and service.

The Hansen’s Disease Unit was created to ensure proper assessment and maximal care of patients diagnosed with this condition.

The Department’s outreach program is conducted as a commitment to serve people. We have several satellite clinics in the city which are now functional. We also have medical missions in the different areas within Metro Manila & in provinces nationwide, such as the Quezon Province, Baguio City, Palawan and the Bicol Region. It has been our priority as a Department to serve our people in the far-flung areas of the archipelago.

The Department is definitely not resting on its laurels; it strives to move on, surpassing difficulties to pursue its teaching goals. On a noble mission, it takes on the challenge with fearless steps & wise discretion guided by its vision of academic excellence & becoming a center for extending exceptional care to its constituents. After all these years, we are proud to say that we have indeed achieved so much, all of which have been realized through the spirit of whole-hearted volunteerism and resourcefulness of the consultant and resident staff. At present, we are all working hand in hand to continuously reach our goal of serving our fellowmen. We look forward to the years to come.

QUALIFICATION OF APPLICANTS:

  1. An applicant must be conferred the degree of Doctor of Medicine in a reputable Medical School
  2. He/She should have passed the Physicians Licensure Examination given by the Philippine Regulatory Commission (PRC).
  3. Following the Administrative Order No. 29 series 1994 of the Department of Health (DOH), the Department strictly adheres to the issuance that residency training in DOH hospitals are strictly for Filipino citizens only. However, foreign nationals are allowed to observe for clinical exposure only.
  4. He/She should be of good moral character, diligent, hard working with pleasant disposition. He/She should be able to work and perform well even under difficult circumstances. An applicant must be willing to accept share of responsibility and quick to extend help among his/her peers.
  5. An applicant should have a well-rounded personality, willing to share one’s talents and skills for the benefit of the department.
  6. The applicant must not have completed a training program in any specialty in a government hospital except Pathology, Pediatrics and Internal Medicine.
  7. He/She must have passed psychological and psychiatric test conducted by the hospital

Makati Medical Center

The Department of Dermatology was organized to serve the dermatological needs of the Makati Medical Center, its charity program and the Center’s Internship Training Program.

The department’s staff of active consultants originally included Dr. Manuel Fernandez, Sr., as chairman, with Dr. Vermen Verallo-Rowell, Dr. Thelma V. Leelin and Dr. Filomena L. Montinola, as senior consultants. Dr. Manuel O. Fernandez, Jr. thereafter joined the staff after training in Dermatologic-Plastic Surgery in the United States, followed by Dr. Imelda Cabral-Villar, the first graduate of the department’s Residency Program in 1992, Dr. Esther Leynes-Ensafian, Dr. Ma. Victoria C. Dizon, Dr. Elsie P. Floreza and Dr. Donna Leelin-Sarrosa.

The dermatology department initiated in 1988 its own 3-year residency program which was promptly accredited by the Philippine Board of Dermatology. To maintain a high quality of training, it limits the number of residents it accepts into the program.

The department continually updates itself by active participation of its staff members in Postgraduate and Continuing Medical Education courses. This is complemented by the acquisition of state-of-the-art technical equipment which first undergo clinical trials for validation of claims. These include the Ultrapulse CO2 Laser, Intense Pulsed Light (IPL) Quantum-SR, Xtreme Clear (Acne Phototherapy System / LHE for Psoriasis / Hair Removal System), VersaPulse C with HELP-G, Daavlin Spectra 311/350, Vela Smooth/Shaper (Infrared-Radiofrequency-Vacuum) and most recently, the Polaris WR System (Radiofrequency-Diode). All these are now housed at the Manuel C. Fernandez, Sr. Phototherapy and Laser Center.

Subspecialty in Dermatology is another strong feature of the department. Guidelines for Applying for the Residency Training Program:

  1. The application forms for the Residency Training Program may be obtained from the Medical Education and Research Division. Completed forms along with required documents will be forwarded to the chairman of the department applied for.
  2. All applicants for residency training shall be reviewed by the Residency Committee of the department where the applicant wishes to train. The composition of such committee shall be made at the discretion of the department chairman. The committee shall have the prerogative to conduct interviews and administer written examinations as methods of screening the applications and choosing the successful candidates.
  3. The residency committee shall forward its recommendations to the department chairman for approval.
  4. The department chairman shall notify the Medical Director of the eligible applicants. The Medical Director shall appoint the recommendees of the department chairman, subject to availability of residency position.
  5. All appointees shall be under the supervision of the Medical Education and Research Division.
  6. All residents are encouraged to familiarize themselves with the hospital policies as provided by Medical Education and Research Division and specific departmental policies prior to the start of training.
  7. The appointee shall report to the Industrial Health Clinic for his pre-employment Physical Examination and to the Metropolitan Psychological Society (Ground Floor 11) for psychological evaluation as scheduled by the Medical Education and Research Division.
  8. Appointees must submit the requirements to Medical Education and Research Division within one week from advice. Delay in submission of the requirements and in undergoing the needed tests for your pre-employment physical examination will also delay the start of your employment with the hospital. When all the requirements have been completed, the appointee shall sign the contract for residency at the Personnel Office (Basement). This shall be recorded as the official commencement of residency training. All records will be held on file by the Personnel Office.
  9. The following requirements must be submitted in authenticated photocopies before any application is officially entertained:

Jose R. Reyes Memorial Medical Center

Department of Dermatology

The department’s origins date back in 1956 as the SKIN & TUMOR CLINIC under the San Lazaro Hospital, then to the LEPROSY RESEARCH & TRAINING SERVICE (LRTS) Division of Sanitaria of the Bureau of Disease Control, Ministry of Health in 1957.

In 1975, the office of the Minister of Health realized the need for health care providers to be more exposed to dermatology, because of the rising number of skin diseases other than leprosy. Dr. Perpetua D. Reyes-Javier, solicited the help of Dr. Lourdes Romualdez for the creation of the DERMATOLOGY RESEARCH & TRAINING SERVICE (DRTS). A proclamation converted LRTS to DRTS with Dr. Romualdez as director, assisted by Dr. Javier.

The DRTS took care of the dermatology and leprosy cases in Metro Manila as well as referrals nationwide from the government and private sectors. It started training courses in dermatology for the medical and paramedical staff, an in-service program equivalent to residency training in dermatology for young graduates, basic courses in mycology and dermatopathology, basic researches on epidemiology.

The People Power Revolution in February 1986 brought about radical changes in the bureaucratic structure. Executive Order No. 5 abolished DRTS, it being absorbed by Jose R. Reyes Memorial Medical Center (JRRMMC) as its newly created DEPARTMENT OF DERMATOLOGY. When Dr. Javier retired as director of DRTS in 1986, Dr. Ma. Luisa Abad-Venida, taking over as Officer-in-Charge, rallied to maintain the functions of DRTS under the new set-up.

In 1988, under the leadership of Dr. Venida as department chair, the JRRMMC dermatology residency program was accorded formal recognition by the Philippine Society of Aesthetic Medicine (PSAM), now Philippine Society of Cutaneous Medicine Inc. (PSCM). Two years later, in October 1990, three fellows of the Philippine Dermatological Society (PDS), namely: Dr. Jesusa Barcelona-Tan, Dr. Lillian Lopez-Villafuerte, and Dr. Elizabeth Amelia V. Tianco, joined the consultant staff to prepare the accreditation of the department to the said society. One month later, the residency program was formally accredited with the PDS.

Under the guidance of the former department chairs, namely Dr. Venida (1986-1998), Dr. Lillian Lopez-Villafuerte (1999-2001), Dr. Jesusa Barcelona-Tan (2002-2004) and under the leadership of the current chair, Dr. Elizabeth Amelia Tianco, the JRRMMC Department of Dermatology continues to accomplish its partnership with the government in providing expert care for patients suffering from skin diseases and in training competent dermatologists for the service of the Filipino people.

The University of Santo Tomas , the oldest university in the Philippines , was the first in the country to recognize the importance of dermatology as a specific field in medicine. As early as 1883, “Dermatosis y su Clinica” was taught as a subject to the sixth year medical students under Sr. D. Felipe Bueno y Chicoy.

It was in 1927 when the Department of Medicine of the UST Faculty of Medicine and Surgery was formally organized by Dr. William Burke, “Dermatology & Syphilology” was formally established as one of its sections with Dt. Jose Hilario as the first section chief.

Section becomes a DEPARTMENT and DIVISION:
By 1988 of March, the section Dermatology became an autonomous Department of Dermatology of the UST Faculty of Medicine and Surgery under the deanship of Dr. Tito Torralba. It likewise became the Division of Dermatology of the University of Santo Tomas Hospital. Dr. Lina De Dios Torralba became the first Chairman with Dr. Rosario R. Caňete as faculty secretary. Dr. Marita Lim Ke was the first residency training officer.

The Santo Tomas University Hospital has a three-year Dermatology residency training program that was given full accreditation since 1981 up to the present by the Philippine Dermatological Society.

Through a written examination and interview process, qualified applicants are chosen for a one-month pre-residency program that starts in mid-September and ends mid-October. The pre-residency period serves as an introductory program on the policies of the department as well as an observation and assessment period by both the residents and consultant staff, hoping that the prospective residents are of high quality, both academically and personally. Regular residency starts in January.

The Dermatology program in the University of Santo Tomas Hospital is designed to mold residents into the best academic and clinical dermatologists with comprehensive understanding of the diagnosis, pathophysiology, histology, medical and surgical management of dermatologic problems, both in the medical and cosmetic aspect of the specialty.

The hospital has both a Private Division and Clinical Division, the latter which is where the Dermatology Out-Patient Department is located for residents supervised by consultant staff of the hospital trains and serve indigent patients to avail of dermatologic care. The residents see the Dermatology patients of both private and clinical division of the hospital. The department also has its own three-bed capacity “Dermatology Ward” at the clinical division where residents directly manage difficult in-patient cases.

Residents rotate with internal medicine and surgery to complement their dermatologic training in clinical dermatology, mycology and dermatopathology. As part of the training, senior residents are assigned specific role in teaching fourth year medical students who rotate through the Department of Dermatology. Lectures on research methodology and critical appraisal are also given to develop skills in research and investigative studies. Combined conferences and in-patient referrals with other clinical departments offer the opportunity for exchange of new information and the chance to discuss the dermatologic aspect of cases of interest with other disciplines and sub-specialties in medicine.

The ” Dermatology Center ” which houses the phototherapy and the iontophoresis machine, as well as the intense pulsed light machine for patient’s use. Patch testing, wound management and tar baths are also done in the center.

All the consultant staff of the Department of Dermatology actively participates in the teaching process of the residents. The present seven residency training staff members are all active Fellows of the Philippine Dermatological Society (PDS). Some have finished fellowship training in subspecialties of dermatology such as dermatopathology, pediatric dermatology, geriatric dermatology and occupational contact dermatitis thus further enriching the training program of the residents. Since 1981 the department has graduated more than 50 resident physicians.

Since 1983, the Division has also committed itself in giving yearly par excellence two-day postgraduate course in dermatology. Well known dermatologists all over the world have served as speakers to share the latest in dermatology to our Filipino physicians yearly.

    Davao Medical Center

    Department of Dermatology

    Davao Medical Center , a government hospital, is the main referral center in Mindanao . Initially, Dermatology was a section of Internal Medicine which only had one consultant, Dr. Carlos R. Pacheco. In 1994, Dr. Ma. Pacita S. Belisario and Dr. Lalaine R. Visitacion volunteered their services to the hospital. A year later, Dr. Ma. Assumpta Cecilia R. Serrano joined the group. Thus, in 1996, the decision was made to start a residency training program with one resident physician initially. This section eventually became a department and was given accreditation on probation by the Philippine Society of Cutaneous Medicine in 1999, pending the presence of a dermatopathologist in the staff.     Making a Choice

    Summer is here again! Swimming pools and beaches are now populated again with people who want to cool themselves from the summer heat. However, because of the increasing awareness of the sun’s harmful effects such as sunburn, wrinkles, photoaging and skin cancer, we try to protect ourselves by using sunscreens.

    There are a great number of available sunscreens now in the market. Sunscreens come with low or high SPF. It has different vehicles, too such as wax stick, spray, gel, lotion and cream. Some are water-proof and some are not. Because of this, consumers find it difficult to choose which among the sunscreen they should purchase.

    We now discuss some of the common questions most consumers ask about sunscreens.

    Which among the available sunscreens is best for outdoor sports and swimming?

    The best sunscreen for swimming and outdoor sports is water-resistant or waterproof sunscreens with high sun protection factor (SPF). It is different from ordinary sunscreens because this type of sunscreen adheres well to the skin and seeps into its outermost layer. Also, this sunscreen has high SPF value (30 or more) and contains water-insoluble chemicals to prevent it from being washed off easily upon contact with water.

    Remember that waterproof sunscreen should be re-applied every 40 to 80 minutes upon immersion in chlorinated or ocean water because it can only protect the skin for that length of time.

    What SPF will give me the best protection from the sun?

    Different SPF gives different percent of protection from the ultraviolet rays of the sun. In Table 1, we can see the SPF with there corresponding percent of ultraviolet absorption.

    It is recommended that we use SPF 15 or higher to protect our skin against sunburn, skin tanning and potential problems like skin cancer and photoaging.

    What are the important ingredients that I should look for in a good sunscreen?

    Sunscreens use a combination of two to six chemical ingredients. And these chemicals have different properties that contribute to its photoprotection.

    There are five types of sunscreens based on the chemical ingredients. These are:

    Type 1: UVB- absorbing sunscreen formulations

    Type 2: UVB and UVA absorbing products

    Type 3: High SPF Sunscreen products

    Type 4: UVB and UVA combination plus particulate sunscreens with high SPF

    Type 5: Physical or Particulate sunscreen

    Among the five types, the type 4 formulation ( UVB and UVA combination plus particulate sunscreens with high SPF ) is the most effective sunscreen against UVB and UVA radiation.

    Type 4 sunblock contains the following:

    Type 1 and 2 chemical ingredients (PABA, salicylates, cinnamates, anthranilates, etc);

    Titanium dioxide and/or zinc oxide;

    Benzophenones (Avobenzone); and

    Enriched formulations can be made with water-resistant chemicals that protect from water immersion.

    Can you give me guidelines on how to use a sunscreen?

    Apply sunscreen 30 minutes before sun exposure.

    Avoid any contact with clothing after application of the sunscreen because this may remove the sunscreen film applied to the skin.

    Use SPF 15 or higher on all exposed areas of the skin.

    Use water-resistant or waterproof sunblock if sweating profusely and if swimming.

    Reapply sunscreens every 1 ½ hours especially if you are immersing in water or sweating profusely.

    Sunscreens are protective from the damaging effects of the sun but it has its limitations. Together with it, we also need to avoid sun exposure by seeking shade whenever possible. Avoid sun between 10 am and 4 pm where the greatest sun damage occurs. Wear protective clothing (with tightly woven fabrics) and wide-brimmed hats.

    Have sunburn-free summer.

    Enjoy the rites of summer, protect yourself from the sun

    The sweltering heat and the sun’s “burning rays” are a given, especially during summer.

    Thus, summer outings should not only be a one-time trip to beach resorts or swimming pools, if only we know how to “tame the sun” and thus avoid sunburns and prickly heat or bungang-araw.

    To enjoy and make the most of family or office weekend outings, here are some trivia and valuable tips from the Philippine Dermatological Society (PDS), the only specialty society for dermatology accredited by the Philippine Medical Association and the Philippine College of Physicians .

    Exposure to direct and reflected sunlight invariably causes skin damage. Sunlight produces ultraviolet (UV) radiation, which comes in two forms, A and B. The latter or UVB rays are known as the “burning rays” and are responsible for skin burns, while UVA rays penetrate deeper and are more responsible for tanning or pigmentation. Furthermore, chronic repeated exposures may cause the skin to have a wrinkled, leathery appearance and all other signs of premature aging. Both chronic exposures to UVB and UVA are risk factors for the development of skin cancer.

    Thus, don’t take the sun’s rays for granted. Always protect your eyes and your skin, the PDS stressed.

    Here in the Philippines and other tropical countries, the sun’s rays are harshest between 10 am and 3 pm. So, don’t expose yourself too long during this five-hour period, without any protective clothing, sunglasses, or wide-rimmed hat and sunscreen.

    When doing outdoor chores such as tending the garden, preferably wear dark-colored pants and a long-sleeve shirt. Dark clothing may be hot, but it prevents the sun’s radiation from penetrating the skin. On the other hand, light-colored clothes absorb up to 80 percent of solar radiation, and reflect only 10 percent.

    It is advisable to apply a broad spectrum (protects against UVA and UVB) sunscreen, preferably with a sun protection factor (SPF) of at least 15, on the skin 30 minutes before doing any outdoor activity. When going out for a swim, skin experts recommend using a waterproof sunscreen lotion, when available.

    Without protection from the sun’s burning rays, the skin’s outer layer turns red and in worse cases develops blisters. After several days, the dead skin peels off. In severe exposure, one may develop sunstroke, which at times is accompanied with vomiting and fever. In worst cases, the sun-stroked patients faint.

    Expectedly, the longer one stays under the sun, he or she has more chances of getting sunburns, said PDS. When longer sun exposure is expected, use sunscreens with higher SPF. However, we should not let the SPF of a sunscreen give us a false sense of security since if we stay out in the sun long enough, we will eventually burn, even with the “best” sunscreen.

    For mild sunburn, you may simply apply a cold compress on the affected area. In severe cases, patients are advised to consult a dermatologist who will give medicines to reduce the swelling, pain and prevent infection.

    But sunburns can be simply prevented by limiting direct exposure to the sun during the harsh 5-hour window, from 10 pm to 3 pm. And when undertaking any outdoor activity, one should apply sunscreen evenly on all exposed body parts, every two hours. When swimming, use waterproof sunscreen and re-apply after toweling off.

    Definitely, summer will not be fun, wholesome and complete without the children and teenagers. And parents should accord them with special care, as their skin is soft and tender, and thus gets easily sunburned. In fact, their skin is six times more sensitive to sunrays than those of adults.

    Another ordinary skin ailment, especially during hot and humid weather, is prickly heat (or bungang-araw ). The most common of which is miliaria rubra , where the pores of the sweat glands get plugged or blocked, resulting in tiny pink or red bumps and even water blisters. The bumps develop on covered parts of the skin — especially where there is friction from clothing or other paraphernalia — on the forehead (when wearing a visor), body folds, upper back and chest, and armpits.

    Often, prickly heat causes irritation and itching, which can be treated with gentle cleansing of affected parts, and thereafter applied with anti-perspiration powder. Patients may also undergo hydrotherapy or take an alkaline bath, that is, soaking for at least 30 minutes to one hour in a tub filled with lukewarm water mixed with one cup of baking soda.

    In severe conditions or widespread itching, patients are advised take a cool bath with cornstarch and/or oatmeal. Another method is applying on affected parts topical antipruritics or itch relievers that contain either calamine, aloe, menthol, camphor or eucalyptus oil and similar ingredients.

    After an outdoor activity, immediately remove sweat-soaked clothes, and towel off. After a couple of minutes, take a cold shower or bath. Thereafter, wear loose, comfortable clothes and apply anti-perspiration powder.

    Aside from sunburn and prickly heat, PDS said that we should also be cautious of bacterial and fungal infections, as these proliferate or multiply faster in warm and humid temperatures. These include athlete’s foot and ringworm and other similar infectious skin diseases.

    Thus, when going to beaches and public pools, wear slippers when taking showers before and after a swim to avoid contracting skin diseases.

    Further, be watchful of skin blemishes that don’t seem to heal despite medication, or moles that change in size, shape or color. These may be indications of skin cancer. And those with a family history of skin cancer should be extra vigilant, the PDS warned. It is advised that patients should immediately consult a dermatologist, who may perform a skin biopsy, which is a quick and painless procedure, to determine if the blemish or mole is cancerous or not.

    Finally, to enjoy any indoor or outdoor activity this summer, the best and simplest thing to do is to observe proper hygiene, PDS noted. If possible, take a short, quick shower twice a day to cleanse away unwanted perspiration and bacteria on your body.

    s sun exposure cannot be avoided, protect yourself appropriately. Put on your sunscreen, wear your favorite sunglasses, a cap or wide-brimmed hat, or better yet open your umbrella — and enjoy the rites of summer, either taking a stroll, a quick dip in the pool or enjoying a beach party.
