-               Original and one copy of the article

-               Electronic copy of the article in a CD specifying file names of  text, tables, and figures (once accepted for publication)

-               Title page

-               Title of article

-               Full name(s), highest academic degrees, and affiliations of  author(s)

-               Name, address, business telephone, cell numbers and fax  numbers, e-mail address of author to whom correspondence should be sent

-               Abstract (double-spaced)

-               References (double-spaced) on separate sheet

-               Tables (double-spaced) on separate sheet

-               Legends (double-spaced) on separate sheet

-               Black-and-white illustrations, properly labeled (two sets of  glossy prints) or Color illustrations, properly labeled (two sets of glossy prints) or pictures in jpeg or gif format saved in CD, properly labeled

*              Digital images such as photos from digital cameras and scanned images should have a minimum 100kb JPEG in size

-               Patient consent letters (photographic and informed consent)

-               Permission letters to reproduce material published previously

* The manuscript should be accompanied by a submission letter/cover letter from the author who will be responsible for correspondence regarding the manuscript.  The cover letter should contain a statement that the manuscript has been seen and approved by all authors.  If the author is a resident physician, the supervising consultant must also read and approve the manuscript, as indicated by his/her signature in the submission letter. The letter should give any additional information that may be helpful to the editor, such as the type of article that the manuscript represents in the journal, information on publication of any part of the manuscript, and whether the authors will be willing to meet the cost of reproducing additional color illustrations.  Include copies of any permission needed to reproduce published material or to use illustrations of identifiable subjects.
