The Erudite: Transformative Dermatological Skills in Clinical Practice

The Jose R. Reyes Memorial Medical Center Department of Dermatology, in cooperation with Skin Research Foundation of the Philippines, Inc. recently held its 17th Postgraduate Course entitled “The Erudite: Transformative Dermatological Skills in Clinical Practice” last July 5, 2019 at the Grand Ballroom, Crowne Plaza Manila Galleria. The 17th postgraduate course aims to disseminate the culture of learning, providing an exceptional and holistic approach in the diagnosis and management of dermatologic diseases. The lectures were imparted by The Erudites, which are considered as persons with profound knowledge, scholarly and wise in the field of dermatology. It is essential, especially in the practice of dermatology that the best and up-to-date clinical practices shall be provided to our patients in these transformative times.

            The convocation started with a welcoming message to all delegates from our esteemed Chairperson, Dr. Ma. Flordeliz Abad-Casintahan, followed by morning lectures by the Erudites, moderated by Dr. Zharlah Gulmatico-Flores and Dr. Edessah Sanchez-Dipasupil. Dr. Mara Therese Padilla Evangelista-Huber discussed about systemic drug prescriptions in special populations, specifically pregnant patients, immunocompromised patients and patients with other co-morbidities, giving emphasis on the safety and efficacy of these medications. Furthermore, disease pathophysiology, drug pharmacology, including its risks and benefits, and patient’s profile should be highly contemplated in the management. Dr. Mary Jo Kristine Bunagan discussed the end of line treatments for difficult to manage cases of hair loss using pharmacologic and office procedures, including Methotrexate, Simvastatin plus Ezetimibe, Liquid Nitrogen Cryotherapy, Calcipotriol, oral or topical Finasteride, and supplements including Saw Palmetto Berry extract, Zinc, Vitamin D and Vitamin E.  Dr. Carolyn Jean Chua-Aguilera refreshed our knowledge in basics of nail anatomy, instrumentation and techniques, essential in performing nail surgeries. Moreover, importance in patient selection and preparation, adequate anesthesia and achieving hemostasis during nail surgeries was expounded.  Dr. Marie Socouer Medina-Oblepias shared her medley of different chemical peels for management of skin disorders on non-facial sites, including Salicylic Acid, Jessner solution, Retinoic acid, Glycolic acid, Trichloroacetic acid, and AZE combination peels, which offers effective treatment, with low cost and excellent clinical results. Dr. Miriam Dumandan-Adraneda shared different conglomerations of therapeutic modalities and approaches for patients with acne scars, which included resurfacing (microdermabrasion, chemical peels, microneedling, and ablative lasers), lifting (subcision), volumizing (dermal fillers, platelet-rich plasma therapy), tightening (fractional or nonablative lasers, fractional radiofrequency) and surgical removal (punch) techniques.

            After enlightening lectures in the morning, everyone had their sumptuous lunch, followed by afternoon lectures moderated by Dr. Abelaine Venida-Tablizo and Dr. Lillian Lopez-Villafuerte. Dr. Hester Gail Lim-Bueser thoroughly discussed the cutaneous effects of radiation from different spectrum of wavelengths, including ultraviolet light, visible light and infrared light. It was recently discovered that visible light and infrared radiation have its mechanistic effects largely linked to wavelength, irradiance and patients’ skin types; although, larger scale studies are needed with use of devices.  Dr. Rosalina Nadela shared her winning combinations against skin aging thru the use of medications and devices. Minimally invasive treatments have been explored and integrated, including resurfacing, volumizing and muscle relaxing techniques. Dr. Allen Aguinaldo-Cabrera presented an interactive lecture regarding prevention and management of complications encountered by dermatologists practicing dermal fillers and neuromodulators, such as hematoma, infections and vascular compromise, which kindled and stimulated the minds of the delegates.  Dr. Melanie Joy Doria-Ruiz commenced The Filipino Skin Lecture Series with 3 challenging and interesting cases, which included exfoliative psoriasis with concomitant bullous pemphigoid, granuloma facialem, and Langerhans Cell Histiocytosis. Dr. Romulo De Villa presented an approach to optimize skin nutrition and holistic management of patients with inflammatory skin disorders, using plant-based diet and avoidance of foods which contain toxins or noxious chemicals. The convocation was wrapped up with a message of gratitude and appreciation to all delegates and the organizing committee from our esteemd Vice-chairperson, Dr. Daisy King-Ismael.

            Countless pertinent clinical pearls and take-away messages were imparted by the Erudites in order to enlighten the delegates. The JRRMMC Dermatology family hopes that this enlightenment, bestowed by the Erudites, shall be used as building blocks of knowledge and skills, to foster and nurture them in order to providing high quality skin care, treatment and management for the Filipino patients. It was a successful postgraduate course, filled with exchanges of intellect and insights. We look forward to see the delegates in the near future, to become the Erudites in their own medical practices.
